Pharmacist - Inpatient Pharmacy (Part-Time)

The Inpatient Staff Pharmacist verifies inpatient prescriber orders, dispenses ordered medications, compounding IVs, and sterile products, delivers timely preparation and distribution of patient orders, provides drug information and clinical patient review for safety and accuracy, and supervises technicians and interns. All inpatient staff pharmacists provide support on patient codes and emergency room level I traumas. Inpatient Staff Pharmacists demonstrate competencies defined in Inpatient Staff Pharmacist duties and responsibilities in providing age-appropriate care to patients from the age of birth through 21 years and select adult patients.

 

➤ Responsibilities

●Dispenses inpatient medications pursuant to prescriber orders. Determines the appropriateness of medication orders using patient information and pertinent past medical history and contacts prescribing practitioner with concerns, reviews all possible adverse drug events, and considers patient allergies. Verifies medications, assures timely and accurate delivery, and appropriate form. Helps operationalize recommendations through order verification as appropriate and as determined by team leadership.

● Provides information to health care practitioners, caregivers, or patients on the appropriate administration of medications, medication dosing, and medication selection. Documents direct patient care, clinical, operational, and financial outcomes, and activities appropriately in the medical record.
● Focuses on accurate maintenance of the "prior to admission" medication list and ensures accurate and timely communication to multidisciplinary team, ambulatory pharmacy, and patient/family regarding discharge medication-related needs.
● Provides in-service training to medical, nursing, and pharmacy staff. Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure. Provides appropriate oversight and input in performance evaluations.
● Provides all necessary pharmaceutical care in support of codes and Level I traumas.

➤ Qualifications

● Bachelor's Degree from an accredited college of pharmacy, required.
● Doctor of Pharmacy, preferred.
● Hospital experience or internship, desirable.
● Talking on Telephone or in Person Constantly
● Typing on the Computer Keyboard Constantly

Pharmacy Intern - Outpatient/Ambulatory Pharmacy

Welcome to Nationwide Children’s Hospital. The Outpatient/Ambulatory Pharmacy Intern position exists to allow for the growth, development, and education of students enrolled in a college of pharmacy. The interns further the mission of the pharmacy department by caring for patients, while enhancing their educational experience, and developing good customer service skills. Demonstrates competency in providing age-appropriate care to patients from the age of birth through 21 years, and select adult patients.

➤ Responsibilities

●Accepts prescriptions, establishes billing/patient charge, documents patient’s allergies correctly, interprets and enters outpatient prescription into the computer

● Prepares and packages injectables and oral medication for outpatient use

● Utilizes math and problem solving skills (i.e. algebra) to calculate amount of drug needed for preparation of prescriptions

● Dispenses medication and counsels caregivers on the medication. Manages dispensing information through the computer system including the patient’s profiles. Reviews patient’s profiles before dispensing medication for possible medication complications and takes appropriate action to correct problems. Complies with OBRA-90 regulations

● Provides drug information to health care professionals, patients and parents; instructs patient/parent on correct use of self-administered medications

● Maintains drug records required by Federal, State, and Local authorities

● Extemporaneously compounds medications which are not commercially available (i.e. suspensions, ointments, etc.) for both inpatient and outpatient use

● Replenishes pharmacy stock area and notifies pharmacist of any problems or concerns

● Answers the telephone and assists caller or forwards to appropriate individual

● Inspects medication storage areas in outpatient pharmacy for expired or unusable medication pursuant to JACHO standard under the direction of an outpatient staff pharmacist.

● Maintains the cleanliness and neatness of the outpatient pharmacy.

➤ Qualifications

● Actively enrolled in an accredited College of Pharmacy

● Eligible for a State Board of Pharmacy Intern License
● Must demonstrate ongoing Inpatient Pharmacy Technician I competencies, knowledge, and skills minimally.
